2. Additional Declarants: Used when multiple parties are making the same declaration
3. Exhibits Reference: Required when the declaration refers to attached documents or exhibits
4. Capacity Statement: Required when the declarant is making the declaration in a specific capacity (e.g., as director of a company)
5. Previous Declarations: Required when this declaration supplements or replaces previous declarations
1. Exhibit A, B, C etc.: Supporting documents referenced in the declaration
2. Identity Documentation: Copies of identification documents of the declarant
3. Authorization Documents: Any powers of attorney or authority documents if declaration is made on behalf of another party
