1. Additional Witnesses: Section for including information and statements from other witnesses who can verify the death, used when the primary declarant's knowledge is not firsthand
2. Foreign Language Declaration: Additional section required when the declarant needs to confirm their understanding of the document in a language other than Danish
3. Medical Professional Statement: Optional section for including statements from medical professionals who attended the deceased, particularly relevant in cases where the death occurred outside a medical facility
4. Previous Names: Section for listing any previous names of the deceased, particularly relevant for individuals who have changed their name
5. Reason for Delayed Registration: Required when the affidavit is being made some time after the death occurred, explaining the reason for delay
